ASW - Social Worker ( 2+ years post qualification experience) - Assessment & Safeguarding
Description
We are seeking an experienced ASW - Social Worker to join the employer’s Social Care team. As an ASW - Social Worker, you will work closely with patients, their families, and healthcare professionals to assess the emotional, social, and psychological needs of individuals.
Responsibilities
Conduct comprehensive assessments to identify the social and emotional needs of patients
Develop and implement personalized care plans for each patient, focusing on their psychosocial needs and goals
Provide counseling and support to patients and their families, addressing issues such as grief, loss, and adjustment to illness or disability
Collaborate with healthcare professionals to ensure a holistic approach to patient care
Connect patients with community resources and support services to facilitate their recovery and well-being
Advocate for patients' rights and welfare, ensuring that their voices are heard and their needs are met
Collaborate with interdisciplinary teams to coordinate discharge planning and facilitate smooth transitions to post-hospital care
Requirements
Bachelor's degree in Social Work or related field
Valid ASW license (or higher) and registration with the appropriate regulatory body
Minimum of 2 years post-qualification experience in a similar role
Comprehensive knowledge of psychosocial assessment and intervention techniques
Strong understanding of medical terminology and healthcare systems
Excellent communication and interpersonal skills, with the ability to build rapport with a diverse range of individuals
Demonstrated ability to work independently as well as part of a team
Proficiency in relevant computer applications and electronic medical record systems
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £24.94
