Summary


An exciting opportunity has arisen within the Staffordshire and Stoke-on-Trent ICB Primary Care Team for a Primary Care Programme Lead. You will join us during an exciting period as we drive forward our Primary Care Strategy and the Fuller Report.

You will support the delivery of the primary care strategy in line with national policy and direction, which includes our delegated function for Pharmacy, Optometrists and Dentists (PODs) across Staffordshire and Stoke-on-Trent ICB. You will take on a strategic leadership role for transformation and contracting across General Practice and the PODs.

This is a home-based office role, with a requirement to travel throughout Staffordshire and beyond for routine work and also as part of regional and national visits

· Responsible for supporting delivery of the Primary Care Strategy and its key priorities which include Health Inequalities, improving patient access to primary care, delivering high quality services and commissioning universal outcomes through Local Enhanced Services. Responsible to delivering the national dental reform, delivery plan.

· Responsible for overseeing the management of primary medical care contracts and services including those delegated commissioned services.

· Responsible for ensuring administration of nationally enhanced services such as the DES, QOF and contract variations.

· Ensure a process is in place for all contract eventualities including breaches, variations, retirements in line with national contract requirements

· Responsible for quality assurance processes for general practice and PODs ensure it national and local milestones and requirements are delivered, working closely with the CQC and NHSE

Develop and implement processes which ensure all patients across the system have the ability to access general primary medical care services

Support the delivery of a major area of the ICBs’s commissioning,service redesign and planning activity, working closely with other portfolio leads to commission services to meet the healthcare needs of the IICB population.

· Ensure that services are commissioned and procured in line with competition law, standing financial instructions and local partnership frameworks

The Staffordshire and Stoke-on-Trent NHS Integrated Care Board (ICB)is responsible for planning functions; managing the NHS budget and arranging the provision of health services in the geographical area.

Location Home based/hybrid working (most of posts).

Travel on occasions to a Hub location with at least 1 days notice



New starters are expected to collect IT equipment, from offices in Stoke-on-Trent, and if based at home are required to have appropriate desk/chair/ relevant equipment in compliance with DSE regulations

Posts at Band 8c and above, will take part in the On-call rota, flexible over 7 days, supporting the System Control Centre, with full training provided.

The post holder will support the delivery of the ICS Primary Care Strategy and the development of the case for change and will work with local system partners to support implementation.

The post is responsible for providing professional expertise to the outcome of the business processes for the Department or Directorate.

The post holder will be the lead for delivering Primary Care transformation framework and schemes. This is to improve clinical outcomes and reduce unwarranted variation between practices and Primary Care Networks. This includes but is not limited to improving patient experience of primary care, delivering high quality services, Improving health outcomes via collaborative working, reducing variation and commissioning universal services and ensuring fit for purpose estate and digital alternatives.

The post holder will act as the lead for the delivery of national plans in relation to dental, pharmacy and optometrists, in particular the national dental reforms.

· The post holder will define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.

The post holder will establish and agree new models of care plans with Primary Care Networks and provider organisations, agree outcomes and a process for evaluation and feedback.

The post holder will line manage the Senior Primary Care Managers

The post holder will contribute to processes and implement those processes and outcomes of the management of primary medical care contracts and services, as well as the delegated commissioning functions.

The post holder will deputise for the Head of Primary Care Contracting and Transformation as required
