This expanded service will be working across the integrated pathway from acute to intermediate care services in the community, supporting patients with the care and rehabilitation they need, improving clinical outcomes and patient experience. To provide specialist physiotherapy to patients; working cross site, rotating between HIT, RADS and CRT as part of an inter-professional team. Participating as a member of the team; managing a clinical caseload within the protocols, objectives and procedures of the team.
This role will require weekend and Bank Holiday working on a rota basis.
To support service improvement and delivery within the team. Participate in health promotion and education of staff, clients and families.
The post holder will undertake regular continual professional development activities.
