Summary


The Merton Case Management Service works to a unique model that recognises that there are varied and complex determinants that indirectly or directly impact on health and well-being. The team uses a wide range of interventions to address these determinants; working closely with both statutory and non-statutory community providers to improve quality of life, wellbeing and promote self-management.



The team combines Case Managers (senior staff with a wide range of clinical and non-clinical backgrounds), a Pharmacist and Care Navigators. Our Care Navigators focus on the Psycho-social background to presenting health behaviours, identifying unmet needs and use the connectedness in Social Prescribing to address these. The Care Navigators liaise with the Voluntary sector, strongly advocates for and liaises with Social Services and other services on behalf of the client and support their navigation through the services that are available to them. As key development goals for the team, support will be given to develop core skills in communication, sensitive assessments to support decision making and enhancing personal knowledge to improve outcomes for patients

· Experience working in health or social care

· Good understanding of community health and / or social services.

· Passion to deliver high quality care to patients with long term conditions and/or varied and complex determinants of health in community settings.

· Willingness to challenge mindsets to support patients to achieve health goals

· Abilityto communicate effectivelybothverballyandinwriting

· Ability to coordinate care across a range of agencies and staff groups

· Be able to learn, develop critical thinking and support new ways of working

· The ability to work as part of a team and also use your own initiative

· Good organizational skills

· Ability to think outside the box
