Summary

The starting salary for this position is £16,256 per annum, working 18 hours per week (prorated from £32,512 FTE).

These are 18-month fixed term contracts with the possibility for extension and/or the roles being made permanent (funding permitting).

We are looking for passionate and creative people to join our friendly and vibrant Communities and Prevention team. As a team we find, create and give support to social care teams and all communities across Surrey.

This role offers a hybrid model of working, with both remote and office-based working. This role will involve approximately 40% working in the office per week.

We currently have opportunities in the following areas:

  • Tandridge - 18 hours per week
  • Runnymede - 18 hours per week
  • Waverley - 18 hours per week
  • Surrey Heath - 18 hours per week

We also have an 8 hours per week opening in Woking but these hours can only be worked in conjunction with one of the above roles.

As stated above, given the small number of hours on offer, it is not possible to apply for only the Woking position and, should you wish to apply for this position, you will also need to indicate your willingness to work 18 hours per week at another location.

About the Team
The Communities and Prevention team work to empower communities and build community capacity, so that more people are participating, engaging and having a role and a say, and no one is left behind.

To this end, we have 6 priorities:

About the Role
We are looking for people who are passionate about building positive working relationships with internal partners and external providers, and who can work alongside a range of professionals and volunteers, including partners across the social care and integrated care system.

You will support an environment where residents with social care needs can be diverted towards informal, free or low-cost community-based support that prevents, reduces or delays care and support needs from developing, as required by the Care Act 2014.

Some of the key responsibilities for this role include:

  • Maintaining databases of information regarding local voluntary sector support, community networks and solutions that are available and accessible to social care teams as well as hospitals and other health care settings.
  • Setting up and working on local and county-wide prevention projects with local partners to develop a range of community assets, upon which residents and communities can draw on to improve wellbeing and increase independence and resilience.
  • Receiving individual referrals from Social Care Teams and supporting practitioners to identify community resources for service users.
  • Working in a strengths-based way, ensuring referral outcomes meet the needs of the residents.
  • Working with voluntary and community groups to build inclusive and self-sustaining models of support, which reduce stigma, promote independence, tackle health inequality, and achieve better outcomes for our residents.

As the role involves travelling across Surrey to community locations that may not be easily accessible by public transport, you will need to be able to demonstrate a flexible and adaptable approach to travelling to those locations. In line with SCC policy, efforts will be made to reduce the travel required where possible. Flexible working will be supported through the use of mobile technology.
