Summary


Brent Integrated Care Partnership has a number of exciting opportunities available, supporting the borough based partnership to deliver its objectives in a changing and evolving health and care landscape. We have 11 roles available, hosted by CNWL but working as part of a wider partnership of health and care organisations in Brent.

About the Roles

Most of the roles available are being advertised on a permanent basis, but others are being advertised on a fixed term 12-month basis, due to changing needs within the organisation over time. The current roles are available on trac jobs are below with reference numbers

This role will identify, develop and deliver a range of transformation activity across Brent’s health and care system to achieve a sustainable and accessible health and care system and an improvement in health and care outcomes across the borough. The role will improve performance by bringing together specialist resources, reduce duplication and improve delivery of care.

This post will be hosted within CNWL but will work with and on behalf of Brent’s Health and Care Partners. The post will contribute to the delivery of the Brent ICP vision and partnership plan and achieve the ambitions of NWL ICB’s Long Term Plan and the Council’s Health and Wellbeing Strategy. The role will also act as a deputy to the Managing Director of the Brent Health and Care Partnership across all aspects of the ICP’s strategy and delivery. Key areas of the portfolio include:

· Children’s services

· Integrated Neighbourhood Teams

· Enabler workstreams

· Governance of the ICP

Brent Integrated Care Partnership (Brent ICP) brings together health and care organisations from across the borough. It aims to work collaboratively with all the health, care and wellbeing organisations that serve the community of Brent.

The partnership includes:

· Brent Council

· London North West University Healthcare NHS Trust

· Healthwatch Brent

· Central London Community Healthcare NHS Trust

· Central and North West London NHS Foundation Trust

· North West London ICB/ ICS

The ICP is dedicated to improving the health and wellbeing of residents and is committed to working with all of our diverse communities to ensure everyone’s needs are met.

We aim to place the resident at the heart of health and care services in order to tackle health inequalities together – working as equal partners and with a collective goal to improve the wellbeing of everyone in Brent.

Because we want the roles to be available to people from a range of different professional backgrounds, these roles are being advertised in CNWL and at Brent Council. The roles will be based at Brent Civic Centre in Wembley, and will involve 3 days working in the office per week, with home working available for the other days.

MAIN DUTIES AND RESPONSIBILITIES

The postholder will be responsible for:

Strategy and Policy Development

· Improving performance by bringing together specialist resources, reducing duplication and improving delivery of care



· Enabling a flexible and adaptable workforce through our investment in education and training for new and current staff



· Aligning workforce planning with strategic and financial planning



· Integrating and improving community services around care in people’s homes

· Improving access to social care services through NHS data sharing



· Increasing engagement with patient groups and with staff to proactively support them to voice their needs



· Hold corporate responsibility for policy implementation both for the ICP and across ICP partner organisations including acute, mental health, community services, general practice, council CYP team and adult social care



· The post holder will need to maintain a good knowledge of emerging policies from government departments. This will assist in the thinking and definition of the strategy discussions for the strategy and stakeholders. In order to lead the development and implementation of the strategic project plan (3 year project plan) for their work stream the post holder will need to:

· Research and identify relevant best practice (i.e. where similar systems are in place), this could be UK wide, globally, public or private sector both within and outside the health economy in order to develop appropriate options to implement policies. Post holder will need to consider how best practice could be applied to the existing health economy, taking into account the impact on existing relationships between entities and their functions across the health economy.
