Summary


Bedfordshire Hospitals NHS Foundation Trust has an exciting opportunity for an exceptional, dynamic Discharge Administrator to join the Integrated Discharge Planning Team based on Maternity Wards at the Luton site.

Working within the Integrated Discharge Team (IDT) you will be part of a team providing support to patients, relatives and staff through admission to discharge.

To work as part of a multidisciplinary team in order to facilitate safe and timely patient discharge from Luton Hospital Maternity Wards. The post holder is the single point of contact in relation to discharge for members of system wide multi-disciplinary teams, patients and their families/carers therefore it is essential that the post holder maintains an excellent working relationship with all stakeholders. Ensure that patients are communicated with on a regular basis and to engage with families when required and prior to discharge. Have an understanding of partnership services available along with criteria for referral or appointment making when necessary. Ensure good working relationships are maintained with all members of the multidisciplinary team. Attend daily Multidisciplinary Team Meetings on wards to facilitate effective Complex and Non-Complex Discharge Planning for both Mother and baby when required. Work on the wards daily to assess and plan discharge requirements. Manage own caseload under the direction of the Senior Midwives. Keep documentation clear, accurate and up to date; include both paper and electronic systems. Patients with complex discharge needs are identified on admission, working with partners to ensure accurate and timely referrals are made when required. In collaboration with the ward staff identify situations that may cause delays in discharge and escalate these to the senior members of staff. Provide daily updates to senior members of staff.
