Summary

Hours currently available- please see below Ideally these will be split between 2 people. Monday 7.45 – 13.00 Monday – 12.45 -20.15 Friday 7.45 – 13.00 Alternate Saturday 12.45 – 20.15 Alternate Saturday night Sunday 7.45 – 13.00 Sundays 12.45 – 20.15 Hours negotiated at interview. What you will be doing: Assist the client to exercise choice and control over their own lives as far as they are able in everyday matters but taking into consideration cognitive problems Liaise with and support the family as required Assist the client to ensure an adequate level of hygiene is maintained Supporting the client to use local amenities such as shops and leisure facilities and attend community events Follow the service user plan as documented for the client Maintain accurate and confidential records Providing distance administration support to progress any ongoing input, Not limited to, contacting third parties, arranging appointments, following up on outstanding actions/tasks Maintain professional boundaries with the client and their family members, adhering to direction from the case manager as appropriate A full job description is at the bottom of this advert.
