Summary

We’re looking for forward-thinking individuals who are experienced in delivering person-centred support, working as part of a team to provide an assertive outreach service to people experiencing the most extreme form of homelessness, sleeping rough. Use your skills, knowledge, and experience to engage individuals needing support to assist them in moving away from rough sleeping and high-risk lifestyle.

We are looking for:

-Experience of working with a range of partners and stakeholders to ensure that individuals identified as at considerable risk of homelessness are supported to manage and navigate the support network available to them, to prevent long term and entrenched rough sleeping.
-You’ll be able to understand and work with adults who may have experienced trauma linked to homelessness, addiction, mental or physical health needs, family breakdown, criminal activity and more, whilst supporting them to engage with stakeholder and community activities that will bring an end to their homelessness.
-Recording and reporting processes appropriate to the role and the department to ensure the safe and effective delivery in all areas of communication on behalf of St Martins.
