Summary


Dorset County Hospital is recruiting a registered nurse to work with Dorset's Targeted Lung Health Check Programme.

The service is a Lung cancer screening programme which invites participants aged between 55 - 74 years old who either smoke currently or have smoked previously for a lung health check.

The NHS long term plan published in 2019 established a plan to diagnose 75% of people with cancer at an early stage by 2028, and this post will support the setting up and implementing of a lung health screening programme across Dorset.

This is a pivotal role, and will support the team to establish a pan Dorset wide Lung Health Check Service by providing administrative support.

The main duties of the role include:

Completing lung health check appointments Mon-Fri 08:30 - 16:30 at different locations across Dorset.

Assessing participants eligibility and fitness to carry out Spirometry at the appointment

Clearly explaining the process of the programme to patients in a way that they can understand

Performing Spirometry once training obtained

The post holder will work within a small team. You will be managing the participant list and will need to maintain accurate records.

The post holder will communicate with patients to deal with all enquiries regarding their appointment times and procedural issues and ensure that patients understand the information they are given regarding preparation for diagnostic tests or procedures and understand the importance of accepting the earliest diagnostic appointment offered.

The post holder will work closely with the Targeted Lung Health Team and Nurses to ensure that clinical patient queries are dealt with by the clinical team.

Completing a national risk assessment. obtaining sensitive patient information which will determine whether the patient is high or low risk of developing lung cancer in the future.

Liaising with the responsible Radiologist, Consultants and Lead Nurse and Co-ordinators to ensure patients are on the correct pathway for follow ups.

Maintaining and updating personal data on patient tracker lists.

Requesting CT scans appropriately.

Documenting findings and escalating appropriately.

Dorset County Hospital is approximately 7 miles away from the coast, and is set in a beautiful rural county.

Plan patient pathways from receipt of participant list from Primary Care to MDT onward referrals.

Work autonomously managing own workload within agreed scope of the role.

Maintain the Lung Health Check patient tracking list, to be responsible for accurately tracking patients through the pathway ensuring as appropriate that the patient is on the correct pathway and has the appropriate next stage agreed and patient is aware.

The post holder is responsible for working as a team member within the Cancer Nurse Specialist Team, working closely with the other Patient Navigators and the MDT Facilitators from the three Dorset acute trusts.

The post holder will communicate with our Primary Care colleagues to request participant list.

Communicate routinely with the Cancer Nurse Specialists, MDT Facilitators and Patient Navigators to coordinate patient pathways between the pan Dorset service

The post holder will establish and maintain effective communication links with all staff within the cancer pathway, including the Cancer Information Team, Cancer Managers and Dorset Cancer Partnership.

Be the resource for the Trust in regards for any queries relating to LHC performance

Coordinate between departments and across trusts pan-Dorset

Resolve issues raised by patients in partnership with the Cancer Nurse Specialists.

The post holder works with other members of the Team to maintain and support departmental/Trust practice and procedures.

The post holder works as part of a team to ensure that an element of cross cover is provided.

All tasks are organised and produced to required standards and within specified timescales in agreement with the Team Leader.
