End of life care and Advanced Care Planning Managing the deteriorating patient with frailty Complex and simple wound care, Observations and ECG's Catheterisation Venepuncture, Utilising advanced physical assessment skills Pressure area care and ordering pressure prevention equipment Comprehensive Falls assessments Taking Medical and Social History Completing Comprehensive Geriatric Assessments (Including Clinical Frailty Scoring and Memory Assessments) Attending MDT's and Broad Rounds Liaising with other community teams and acute trusts Line management of Junior members of Staff Health promotion and education Referring to Therapists for patient rehabilitation
