Summary
Other responsibilities include To use telephone triage to assess patient and family need, giving advice and support. To offer face to face assessment, to provide the best and appropriate care for patients at the end of life which meets their choice and to support their families/carers experiencing the patient's journey alongside them. To take a clinical lead within St. Lukes Hospice Community Services working at a specialist level, and ensuring standards are maintained throughout the team. To be able to provide high standards of nursing care for palliative patients according to their assessed plan of care/need. To assess initially, and onward review to meet the above needs in any setting home, hospital, care home and Hospice. To be fully aware of responsibilities and processes in safeguarding vulnerable adults and children. To promote personal and professional development of all nursing staff.
