Main duties of the job

You will be a vital part of a supportive, friendly and specialist team who have a great reputation for ensuring that patients, families/carers receive seamless support in the community. In this role, you will work flexibly to support and deliver care. You will undertake triage and rapid response visits, manage palliative discharges, assess and review care support, conduct full patient assessments and plan care.You will need to have relevant palliative care experience to be successful in this role and because you will be working out in the community, you needa drivers licence and use of a car/vehicle.

Job responsibilities

Other responsibilities include

  • To use telephone triage to assess patient and family need, giving advice and support.
  • To offer face to face assessment, to provide the best and appropriate care for patients at the end of life which meets their choice and to support their families/carers experiencing the patient's journey alongside them.
  • To take a clinical lead within St. Lukes Hospice Community Services working at a specialist level, and ensuring standards are maintained throughout the team. To be able to provide high standards of nursing care for palliative patients according to their assessed plan of care/need.
  • To assess initially, and onward review to meet the above needs in any setting home, hospital, care home and Hospice.
  • To be fully aware of responsibilities and processes in safeguarding vulnerable adults and children.
  • To promote personal and professional development of all nursing staff.

  • Registered Nurse Qualification and NMC pin number
  • A commitment to work in Palliative Care Nursing
  • Clinical Assessment and Diagnosis Skills
  • Cancer/palliative care experience
  • Community nursing experience
  • Confident IT skills
  • Excellent written and verbal communication skills
  • Flexibility and willingness to work on rotating shift patterns
  • Advanced Non-Medical Prescribing desirable but not essential
