We're excited to offer an outstanding opportunity for a skilled Diabetes Assistant Practitioner or Nursing Associate to join our esteemed Integrated Community Diabetes Service. In this pivotal role, you'll be instrumental in providing crucial support and continuous education to individuals living with diabetes. As a valued member of our Integrated Community Diabetes Team, you'll have the honour of delivering exceptional care and support to adults with diabetes across a spectrum of settings, including MDT clinics, group education sessions, and one-on-one interactions. Whether you come from a background in health or social care or have experience in primary care, we're seeking a candidate with a diverse skill set, particularly in venepuncture, foot surveillance, vital signs monitoring, and providing assistance in clinical environments.

Health and Social Care (Assistant Practitioner) FdSc or Health and Social Care Foundation Degree (Fda) or equivalent is essential for applicants.

n your role as the Diabetes Assistant Practitioner, your primary focus will be collaborating with our community diabetes multidisciplinary team to provide holistic care to patients with diabetes, while also supporting the clinical team with educational group sessions. Your flexibility will be highly valued as you rotate through various clinic locations and conduct home visits within the Heywood, Middleton, Rochdale, and Bury boroughs.

Here, you'll have the privilege of delivering comprehensive care, invaluable support, and essential education to patients, empowering them to effectively manage their diabetes. Under the guidance and supervision of highly Specialist Diabetes Nurses/Dietitians, you'll have the opportunity to grow and excel.
