Summary

We are pleased to be able to recruit a Community Mental Health Nurse to join the Recovery and Wellbeing Team based in Southend on Sea. This is a busy, supportive, well established Multi-Disciplinary Team.

We are seeking a Mental Health Nurse looking to be part of future healthcare and integrated working with the Adult population.





To be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within the Southend on Sea locality.

To provide professional advice and support to other agencies and members of the multidisciplinary team

To act as a role model and resource for junior/less experienced staff.

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

To be responsible for the assessment, planning implementation and evaluation of the evidence based care required, including health promotion for a caseload of service users within the Recovery and Wellbeing service and to beresponsible for the appropriate clinical risk assessment.

To be responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team.

To be responsible for participating in the development of the service, setting of quality standards, including the auditing, monitoring and reviewing in line with current clinical guidance practice and policy.

To ensure promotion of carer and service user involvement within the service and for its provision.

To be responsible for establishing therapeutic relationships with service users, and the implementation of evidence based therapeutic interventions with appropriate boundaries in accordance with Nursing and Midwifery Councils professional code of conduct.

To be responsible for ensuring that carers’ assessments, education and support is delivered in accordance with the requirements of the NSF.

To deputise and provide appropriate support and caseload coverage for colleagues/team members.
