Summary

As a Nurse you'll deliver a broad range of healthcare services to older people with mental and physical health needs, offering long term dementia care and bed based reablement for hospital leavers, so no two days are ever the same. You'll deal with clinical diagnosis and treatment, be responsible for the whole patient experience and build relationships with the people in your care. Our residents are at the heart of everything we do, and our sole aim is to give them the dignity, respect and independence they deserve. In this role you will be responsible for: Provision of person-centred, professional nursing care to residents within Older Adults nursing homes and Short-Term Services. Assessing, planning, implementing and evaluating care plans for a designated number of residents. Maintaining accurate records on resident care and reporting clinical and non- clinical events to assist with internal and external audits. Supervising, mentoring, supporting and educating more junior staff in the home. Liaising with other healthcare professionals and work in collaboration with other members of the multidisciplinary team, ensuring a timely escalation when needed. Promoting good practice to achieve continuous improvements in nursing care and assist with audit trials and bench marking clinical services within the home. Day to day tasks may include: Phlebotomy, injections, syringe pumps, insertion/removal of urinary catheters and removal of sutures. Dealing with wounds. Supporting end of life care. Involving and engaging with residents and their families during the care planning process Safeguarding All employees will be required to adhere to internal safeguarding policies, procedures and guidance, taking action and raising concerns as appropriate. Where a professional code of conduct applies to the role, this must also be adhered to. All employees will be required to have an understanding and awareness of the specific responsibilities placed on individuals who care for vulnerable adults.
