Summary

The following are the core responsibilities of the Data Support Officer. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Other tasks will include Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice Deal with all general enquiries Process personal and telephone requests for appointment, visits and telephone consultations according to practice protocols and guidelines Receive and make telephone calls as required divert calls promptly and appropriately and take messages ensuring accuracy of details and prompt appropriate delivery Using your own judgment and communication skills ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner. Processing and distributing incoming (and outgoing) mail Scanning Preparation of morning visits in liaison with GPs Provide support to reception/telephony clerks by dealing with any patient queries requiring further investigation Provide support to GPs with making telephone calls on their behalf Computer data entry of daily letters Completion of filed pathology results Completion of online referral as appropriate Provide support to the GPs and Assistant Practice Manager in relation to chasing hospital appointments /queries Redirecting letters and pathology for on leave GPs Processing of sick note requests Liaising with community Nursing team, Care Co-Ordinators etc Daily messages for GPs Liaising with Coroner, medical examiner, registry office and funeral homes following deaths Arrange appointments for medicals that are required and agreed with GP Arrange weekly ward rounds for care home residents To be pro active in making arrangements i.e. DNS1 preparation of paperwork for comm nursing team to administer medications accordingly First point of contact for palliative, elderly and vulnerable patients and their families Provide emotional support To undertake any other duties or responsibilities which may reasonably be required Attendance at month community health care team meetings (MDT) Attendance at end of life care meetings (GSF) Triaging/assigning home visits Compiling list for ward round Arranging DVLA medicals Other responsibilities To have a thorough knowledge of all practice procedures To work in accordance to practice protocols Ensure building security have thorough knowledge of doors/windows/alarms Opening up/locking up of practice premises and maintaining security in accordance with practice protocols Computer data entry processing and recording information in accordance with practice procedures Providing clerical assistance to practice staff as required from time to time Undertake any additional duties as requested by the partners or the practice manager Administration Mail to patients and required, i.e. calls and reminders Photocopy as requested Medical record summarising: To review medical records and produce an accurate summary of the patients medical history, within the Practices protocols and agreed timescales. Enter Read Code information onto the Practices computer system. Use medical knowledge to correct mistakes found in existing patient records. Adhere to and monitor progress of notes summarising against Practice targets, maintaining structure and protocols when recording new information. Be aware of specific Read Codes and associated records; following Practice protocols for recording, obtaining and filing patient records as appropriate e.g. child protection. Assist GPs and other Practice staff with queries relating to medical records. Liaise with internal/external departments or healthcare providers as require. Check vaccinations are recorded correctly on notes Child health surveillance: Tracking childhood immunisations and reviews Supporting the Safeguarding lead with administrative tasks) Arranging safeguarding meetings and taking minutes
