Summary


The Greenwich Care Homes Complex Case Management Team are seeking an enthusiastic administrator to join the team. The Service is currently in a pilot stage which provides a comprehensive assessment and care planning to ensure patients receive the optimal health services and support, reducing avoidable non elective admissions to hospital and maintaining care in their home.

To provide efficient and comprehensive administration to the Greenwich Care Homes Complex Case Management Team. This involves processing referrals to the service ensuring appropriateness and accuracy of information. Ensure that all computer systems and databases are up to date with relevant information. Co-ordinate Multidisciplinary meetings between a variety of organisations. The post holder will be expected to act as a central point for information and communication for the team

Successful applicants will have experience in customer-care, preferably in a health or social care setting. As the initial point of contact for telephone callers, you will provide advice and information to members of the public, service users and professionals throughout the health and social care network. A professional, friendly and efficient telephone manner is essential along with an ability to deal with a diversity of people, who could be upset or challenging, in a sensitive and polite manner.

You will need excellent organisational skills to manage a busy and varied workload. An excellent working knowledge of IT packages is essential, as is your experience in working with databases. A working knowledge of RIO or Mosaic would be an advantage. The post will include processing referrals to the service and ensuring information is up to date. The post holder will also take a key role in co-coordinating multidisciplinary meetings between partner organisation’s and ensuring that relevant documents are ready for meetings.
