Summary
To ensure that physical health is viewed on path with mental health needs. Encouraging and collaborating with patients to raise awareness of common physical health conditions that may occur in people with a serious mental illness. Supporting them to overcome barriers to annual routine physical health checks, e.g. blood tests and ECG. Supporting the team to achieve physical health KPIs Responsible for managing physical health care plans and goals Responsible for building relationships and sharing information with PCNs in Medway to support better physical health in our patient group Please see the job description for more details.
