Summary


We are currently recruiting intoafull timeAdvancedNurse Practitioner [ANP] postwithin the inpatient Unit atOakwood Community Hospital, Berkshire.Oakwood Community Hospital is a24 bedphysical health ward within Prospect Park Hospital.


You will, work closely alongside medical staff supporting the delivery of care, which may include the community geriatrician, specialist doctor,GP’sand other multi-disciplinary colleagues.


Whilst this post will bepredominantly basedat Oakwood Community Hospital, the post holder maybe requiredto work at any of our other community hospitals in BHFT (West Berkshire Community Hospital and Wokingham Community Hospital) if needed,to cover shortages that mayarise


You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Working as part of an integrated team, you will be critical in supporting thephysicalhealth inpatientunits and play a lead role in supporting admission avoidance andthe safe discharge of our patients into the community.

• To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting and discharging patients to community inpatient units and onto the Rapid Response and treatment pathway in collaboration with the specialist doctor, Community Geriatrician and/ or GP.

• Provide expert professional advice support and guidance to other members of the health care setting on the care required for patients.

• Develop, monitor, evaluate and review the delivery of the service in collaboration with the Ward Sisters and service manager.

• Contribute to the education of others by guiding and developing Team members in their professional and clinical practice.

• To proactively manage patients with long term conditions and acute exacerbations of these, working collaboratively with the medical and nursing staff and be an advocate for shared care between professionals.
