Summary


An exciting opportunity has arisen within the Patient Flow team at North Manchester General hospital (NMGH). The area of Patient Flow has had significant development and investment in recent years at NMGH and we now have a well established team to ensure that our patients receive the right care, at the right time, in the right place.

Due to retirement there is now a vacancy in the position of Team Leader for Patient Flow. This is a band 7 position and if successful you will have a key leadership and operational role at a senior level and will provide specialist knowledge and skills to oversee patient flow across the Hospital. To do this you will communicate effectively to the wider MDT team and act as a role model, demonstrating a professional and proactive approach with colleagues and the Trust, to support the delivery of high standards of care, ensuring an environment that is conducive to the patient’s well-being. You will support the development and improvement of services and continue to strive to provide the very best care and experience to all patients. Key to this position will be providing line management to the Patient Flow Team , as well as supporting other community services based within the hospital that support patient discharge.

You will need to hold an active registered nurse qualification and have band 6 experiences in a patient flow role, preferably patient discharge . You will be supported by the Head of Patient Flow.

To facilitate the timely, effective, appropriate and safe discharge of patients from the acute hospital setting to the community. To work collaboratively with members of the multidisciplinary team within the hospital, community, social and health services to reduce delays in discharge. To lead the discharge process for a group of individual patients and direct junior colleagues in the discharge process . To provide direct line management to the members of the patient flow team.
