Summary


We are recruiting 1 Full Time (37.5 hrs) and 1 Part Time (15 hours per week) Trainee Care Navigator to join our new Community Diagnostic Centre (CDC) in Harpurhey.

You will join our admin team, covering reception and booking and scheduling patients in for their Diagnostic tests.

The CDC offers a community facing diagnostics service with focus on delivering a seamless interface between primary care, hospital and community health and social care services, ensuring the people of
Manchester have access to the right services at the right time in the right location.

This role is an opportunity to be trained as a Community CDC Care Navigator whereby you will develop to support patients through their patient journey, from beginning to end, by being involved in all aspects
of the patient journey, including outreach into the community.


• Dealing with routine enquiries in a pleasant and helpful manner, communicating relevant information to stakeholders, referring to others as appropriate.
• Answer telephone calls, taking messages and passing on accurately to the relevant departments/person.
• Ensure a high level of customer service through effective and timely communication with patients, managers and staff.
• Work effectively as a key member of a small team Supporting the team with administrative duties in relation to outreach work within local communities.
• Chase referrals, results etc for patients and other duties requested
• To proactively contact patients to pass on information from the clinical staff relating to appointments or information

The CDC opens between 08:00-20:00 Monday to Friday and 08:00- 18:00 Saturday and Sunday.
