This opening assignment is for 4-5 months

Provide Effective Social Work, Prevention, Assessment and Case Management and Reviews of Care and Support of Adults under the Care Act 2014 and other statutory requirements.
Key Responsibilities


Meet and maintain Social Work Registration requirements
Through a Strengths Based Approach support people to live a fulfilling life at home, connected to the community and resources around them, and provide care and support where needed
Assess and understand needs and wellbeing under the Care Act 2014 to determine eligibility, producing written assessments that evidences eligibility and evidences critical reflection and analysis to inform and provide rationale for professional decision making
Provide information or support to facilitate wellbeing of the people and carers of Sheffield
Working with other professionals and the voluntary sector to support the person and carers
Utilise a range of IT software systems to record actions in line with General Data Protection Regulations (GDPR)
Recording rationale for decisions made, co-produce support plans with the person and undertake risk assessment, applying problem-solving skills that n prioritises independence, choice and recovery and utilises community assets.
Work within the principles of the Mental Capacity Act 2005, assessing capacity and working in a person’s best interest where required
Safeguarding, prevent harm and reduce the risks of abuse and/or neglect to Adults with social care needs as guided under the South Yorkshire Safeguarding Protocols
Report writing for the purpose of legal forums. Attendance at appropriate legal forums with support

Qualifications:


CQSW, CSS, DIPSW Social Work Degree or equivalent
Registration with Social Work England

Knowledge:


Knowledge and application of Social Work Interventions
Knowledge and ability to undertake appropriate investigations under safeguarding procedures
Knowledge and application of relevant legislation, policies and guidance
Knowledge of the referral and signposting processes regarding Housing, Welfare Benefits and other community support services
Have a knowledge of and adhere to local and national policy affecting adult social care
A basic knowledge of equipment and adaptations available to promote independence e.g. assistive technology
A practical understanding of care and support needs of people as a result of a physical and/or a mental condition
An understanding of person-centred practice and skills relevant to this role.

Experience:


Social Work experience of working with people and the public, voluntary or independent sector
Experience of multi-disciplinary working

Skills & Abilities:


Effective negotiation skills
Excellent interpersonal and communication skills, both written and verbal at an appropriate level
Ability to be creative and innovative in relation to support networks
Ability to use own initiative, adapt and respond to dynamic situations. Able to self-manage, self-motivate and be accountable for own work
Ability to travel city wide to fulfil the role
Able to work with minimum supervision and make timely decisions to maintain the integrity of data.
Ability to assess needs of people and identify and mitigate risk with skills in assessment and individual care and support planning with people and carers
Ability to create individual plans, understanding responsibilities and tasks associated with developing networks and/or arranging services in accordance with their eligible needs.
Ability to work positively as a member of a team, and to liaise with other colleagues/professionals within the context of the person’s own support networks
Proficiency with spreadsheets and data analysis.
