About Our Locality Teams:

Our dedicated Locality Teams work with adults of all ages, who are experiencing the impacts of short- or long-term health conditions, physical and/or cognitive impairments. We work with their carers to support them too. Our aim is to help people experiencing build on their strengths, achieve their goals, and be connected to their community.

Every day is different for our Assessment and Review Co-ordinators with the job role including assessments of peoples strengths and needs, and support planning with people to help meet their goals, reviewing how someone’s support is working for them, and assisting with actions needed to safeguarding people where a concern is raised.

About You:

We are looking for someone who shares our vision and aspirations for people – someone who wants to make a difference.

As our ideal candidate you will:

Have experience is writing in assessments and/ or risk assessments about people’s circumstances, and experience of creating person-centred plans to support an adult in need of assistance to lead a good life.

Have knowledge and experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, and Making Safeguarding Personal

Be confident in working at pace in challenging situations, to make sound decisions quickly to support those in crisis, and to ensure people are seen and supported in a timely manner.

Have experience of working with health colleagues, voluntary sector organisations, and other partner agencies to help people find solutions to maximise their independence and resilience and enable people to take positive risks and achieve their goals.
