Summary


An exciting opportunity to join the Home Hospice Care Team as an Assessor has arisen.

The Home Hospice Care Team sits within the wider Oxfordshire Rapid Intervention for Palliative and End of Life Care project, as a separate team. The Home Hospice Care Team primarily provides care to people who are likely to die within the next 2 weeks and are at home. The team is also required to provide short term support to people who are in crisis.

The postholder will be responsible for completing initial assessments in the patient's home, as well as reviewing the care to ensure the needs of the patients are being met.

The postholder will also be required to line manage the Patient Support Workers.

The Home Hospice Care Team (HHCT) is looking for an Assessor to join our community based team.

The team operates 7 days a week from 7am-10pm, this includes Bank Holidays.

The team covers the whole of Oxfordshire, so it is essential that the postholder has a full UK driving licence and a reliable car available for work.

HHCT has the following two bases, Unipart Business House and Horton General Hospital. You will predominately be covering Central and South Oxfordshire but do need to be willing to travel across Oxfordshire.
