Summary

Job Purpose To assess Respiratory and Cardiac patients and provide appropriate care including Pulmonary Rehabilitation and Cardiac Rehabilitation sessions for adult clients with a wide variety of complex physical needs in a community setting this may be in clients own homes or in community venues. -based treatment depending on patient agreed goals Determine clinical diagnosis, physiotherapy treatment and exercise prescription indicated and to maintain records as an autonomous practitioner working with supervision. To provide specialist advice, assessment, and treatment for patients with respiratory and cardiac conditions within the community and to support patients in respiratory exacerbation in an aim to avoid unnecessary hospital admissions. To work cohesively with other health professionals/agencies through the respiratory service referral process to ensure effective treatment management and planning for those with a respiratory condition. As an autonomous practitioner you will undertake comprehensive respiratory and/or home oxygen assessment for patients requiring urgent and chronic respiratory disease management or oxygen therapy. To undertake acute visits for respiratory patients needing assessment and advice. To educate and advise patients under the Integrated Respiratory and Oxygen Service patient pathway and those involved with working with the patient including other health and social care workers, students, or carers. To be responsible for continuing the on-going development of high standards of clinical practise and expertise within the service contributing to implementation of specific changes to practice and service protocols and making a professional contribution to the work of a multi-disciplinary team. Dimensions To provide line management and mentorship to clinical staff and peers working within the specialty To work collaboratively and across organizational boundaries to provide evidence-based practice and holistic care to patients in a variety of settings To be responsible for the safe use of equipment and physical resources in the course of clinical duties To be responsible for the delivery of a physiotherapy service to clients within own locality for clients referred from Consultants, GPs and other Health Professionals. To supervise, educate and assess the performance of undergraduate Physiotherapy students asrequired ensuring the standard of practice and teaching meets the standards set by the degree level qualification. Key Responsibilities Clinical To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. To ensure a high standard of clinical care for the clients under physiotherapy management and support more junior staff to do likewise. To work closely with community and secondary care providers to identify and manage patients with a respiratory condition at risk of frequent admission into hospital. To work closely with community and secondary care providers to identify and manage patients requiring Home Oxygen including providing assessment, support, and management of patients on Long Term Oxygen Therapy (LTOT) and Ambulatory Oxygen Therapy (AOT). To provide specialist care and advice to patients with a respiratory condition undertaking full respiratory assessment, chest auscultation and interpretation of findings to manage urgent respiratory exacerbations and ongoing chronic respiratory management in the community/home setting. To develop and co-ordinate individualised patient management plans, including a respiratory self-management plan, in partnership with the patients and their carers/family. To work as a lone practitioner with supervision from the Team Leader and support from other senior members of the team. To provide spontaneous and planned advice, teaching and instruction to clients, carers and other professionals to promote understanding of the aims of physiotherapy and to ensure a consistent approach to client care. To assess, advise and where appropriate provide mobility aids including teaching clients / carers in their use. Utilise enhanced assessment skills and evidence-based knowledge to provide specialist diagnostic service delivery in the clinic setting in relation to accurate diagnosis, interpretation of blood results, blood gases and the management of chronic respiratory conditions To undertake the comprehensive assessment of cardiac patients, including those with a complex and/or chronic presentation using investigative and analytical skills through standardised and non-standardised assessments. To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care and exercise prescription where appropriate. To deliver Pulmonary and Cardiac Rehabilitation courses for patients, including patient education sessions held in a range of clinical settings. To determine by assessment and liaison with other relevant agencies the advice, education, and treatment requirements of each client and or carer. Work in collaboration with primary care teams, secondary care teams and other professions making appropriate referrals where needed including information, support, and referral to the smoking cessation service. Manage complex situations with a vulnerable client group. Respond rapidly to the constantly changing environment and client needs. To continually risk assess situations. To set physiotherapy rehabilitation goals, carry out treatment interventions and/or home assessment visits (often as the sole health professional) To accept clinical responsibility for a designated caseload of clients and to organise this effectively regarding clinical priorities and use of time. To provide highly specialised advice, teaching and training to other members of the Integrated Respiratory and Home Oxygen and Primary Care Cardiac Teams regarding the physiotherapy management of clients with problems related to rehabilitation and long-term condition management. To train, supervise and manage more junior staff, support staff and students. To supervise, educate and assess the performance of undergraduate Physiotherapy students as required to ensure the standard of practice and teaching meets the standards set by the degree level qualification. To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment. Professional To participate in the development of good practice for Physiotherapists within Nottingham CityCare Partnership under the guidance of the Professional Lead Physiotherapist To demonstrate a commitment to evidence-based practice. To be responsible for own Continuing Professional Development under the supervision of the team leader keeping abreast of new trends and developments, incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses. Support more junior staff to do likewise. To maintain a high level of physiotherapy standards and work to the HCPC Code of Professional Conduct. To actively participate in the services training strategy, including in-service training, Trust, and multi-agency training. To participate by attendance and presentation at staff meetings, tutorials, and training sessions. To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to the work situation. To participate in and contribute towards any related research projects that have been formally agreed within limits of time and funding. Ensure that evidence-based findings are translated into practice. To collect and utilize data to enable monitoring and evaluation of the service, which will provide evidence of the value and effectiveness of the service. Supporting the use of clinical audit as a means of enhancing clinical practice and being receptive to the audit plans of others. To participate in the staff appraisal scheme and Personal Development plans as both appraiser and appraisee. Take responsibility for line management within the service. Take a lead role for identified aspects of the service. Organisational To support the Team Leader in implementing targets and objectives to assist in the ongoing development of the Primary Care Cardiac Service. To participate in service evaluation, audit and relevant research and advise the Team Leader on the service needs of the client population. To be responsible for management duties when requested by the team leader. To liaise effectively with other professionals and agencies to enhance rehabilitation and cardiac physiotherapy within Nottingham CityCare Partnership. To manage the day-to-day delivery of own caseload within the physiotherapy service including prioritisation of support staff duties. Also organising own diary and time management. Arranging assessments and appointments for own caseload. Participate in the interview process and recruitment of team members. To always ensure the health & safety of staff and clients in accordance with Health & Safety legislation and Trust policy and procedures. To adhere to all other Trust Policies and Procedures. Ensure that confidentiality and data protection is always adhered to. To attend team meetings and meetings with other agencies as required. To use information technology daily and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy. To act as a Duty Clinician and Duty Officer for the Lone Worker Tracking procedure. (Checking all staff have safely returned from visits and if necessary, reporting missing clinicians to the Team Leader on call at the end of the working day) To undertake any other duties which may be reasonably required.
