Job summary

An exciting opportunity has arisen for a Band 6 Physiotherapist to join Nottingham CityCares Integrated Respiratory and Home Oxygen Service and Primary Care Cardiac Service on a permanent contract working 37.5 hours per week across the two services.

Both services are forward thinking and dynamic and provide evidence-based assessments and follow ups for individuals requiring urgent and routine symptom management and provision of various therapies in the community as well as rehabilitation and education in both home and clinic settings.

We are looking for an enthusiastic, motivated and conscientious physiotherapist with respiratory and cardiac experience. The teams will provide the successful applicant with the opportunity for postgraduate and specialist training to enable them to build on their current respiratory and cardiac skills enabling them to support patients to be independent in their community.

The physiotherapist core hours are between 8am and 5pm Monday to Friday.

Main duties of the job

You will undertake routine clinical management of patients with respiratory/cardiac needs in an integrated manner within the service pathway. Within the wider service you will be involved in audit, development of practice and take an active role in case management.

You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritize tasks and manage time effectively, also be able to work in an integrated manner and flexibly within the team and be confident in decision making. You must be willing to undertake specialist training as required. You will need to hold a valid driving license and have access to a vehicle for this role.

  • To develop and co-ordinate individualised patient management plans, including a respiratory self-management plan, in partnership with the patients and their carers/family.
  • To work as a lone practitioner with supervision from the Team Leader and support from other senior members of the team.
  • To provide spontaneous and planned advice, teaching and instruction to clients, carers and other professionals to promote understanding of the aims of physiotherapy and to ensure a consistent approach to client care.

  • To deliver Pulmonary and Cardiac Rehabilitation courses for patients, including patient education sessions held in a range of clinical settings.
