Summary


Central Pre assessment clinic at GSTT is currently recruiting into our new and exciting Band 7 Clinical Nurse Specialist.

Pre assessment clinic is undergoing a transformation with this new post. We are looking for an experienced nurse with an expert knowledge of pre assessment. You will be running your own face to face clinic autonomously, prioritising your workload and assisting in meeting NHS waiting list targets for urgent / cancer pathway patients. You will be teaching and supporting in signing off pre assessment competency for new staff and promote a learning and teaching environment for all staff. You will also be managing a group of staff to achieve above goal.

Your role will also involve reviewing adult sleep oximetry studies and referring patients to relevant sleep clinic for treatment. Training will be provided.

You are expected to have a hybrid role between nursing assessment and the high-risk anaesthetic clinic. You will be supporting the anaesthetist in managing their high-risk complex patients.

As this is a new post, you will assist the Matron and affected specialties in mapping out a workable pathway for the urgent / cancer patients’ referral that needs pre-assessment in a timely manner.
Main duties of the job

This post will be a hybrid role covering both nursing pre assessment and the high risk anaesthetic clinic.

The post holder will:
• Assess high risk patients awaiting surgery and request appropriate investigations in a timely manner
• Support the Pre-assessment team with the complex patient caseload.
• Support the development of appropriate and timely pathways for complex patient management, whilst implementing and evaluating a seamless service ensuring patients receive the highest standard of clinical care.
• Develop and promote an environment of learning and teaching and will be able to communicate the risk to patients having surgery.
• Work independently as well as working with the Anaesthetic and the Pre-assessment team in caring for patient in the outpatient setting
• As a pre assessment specialist nurse, you will demonstrate a high level of expertise within the pre assessment, providing specialist advice, education and support to health care professionals and patients.
• You will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols, guidelines and contribute to service developments in collaboration with multidisciplinary colleagues.
• You will ensure the safety and wellbeing of service users by working in a manner that promotes respect, dignity and person centred care principles.

To work autonomously, managing a caseload of patients within pre assessment whilst working as part of the multidisciplinary team, delivering
individualized and personalised direct patient care.

Be responsible for assessing, recognising and managing emergencies in
pre assessment. To interpret information, take appropriate actions and to lead others to do the same.

Act as an advocate for patients within the department, to ensure a patient
orientated approach to the delivery of care and to meet standards within
the NHS Plan and other relevant guidelines.

Ensure that patients’ receive high quality clinical care and a good patient
experience, having regard for equality, diversity and inclusion.

Recognise and avoid situations that may be detrimental to the health and
wellbeing of the individual.

Actively engage service users through the facilitation of user groups and
open days.

Co-ordinate the provision of specialist services to patients, both within
hospital and/or in the community setting, effectively liaising with
multidisciplinary colleagues.
