Summary

A Vacancy at Hounslow and Richmond Community Healthcare NHS Trust.


We are looking for a talented and dynamic occupational therapist for this newly created post in our Falls Prevention and Bone Health Service.

You will work as part of a multi-disciplinary team, including physiotherapy, dietetics and nursing, with patients and their carers to offer a multi factorial falls risk assessment followed by a range of multi-disciplinary interventions. You will also work closely with colleagues in the acute Falls / Frailty clinic, the local authority home safety service and voluntary sector services. We are seeking someone with a passion for Occupational Therapy along with the skills to work collaboratively and creatively as part of the multidisciplinary team.

The service is delivered through multiple site clinics and home visits. The post holder will work flexibly and be mobile across the Hounslow Borough.

Provide multifactorial falls risk assessments and comprehensive fracture risk assessments and bone health management for clients who are at risk of, or worried about falling.

Independently manage a caseload without direct supervision and to be professionally and legally accountable for all aspects of own work, including the management of clinical risk.

Apply knowledge and skills in the assessment of occupational performance in order to formulate preventative intervention and promote self management for people at risk of falling.

Deliver group and individual interventions, working as part of the multidisciplinary team to prevent falls

Comply with the Royal College of Occupational Therapy Core Standards of Practice and Health Professionals Council Standards of Performance Conduct and ethics of all other relevant national and local policies and procedures.
