Summary

A Vacancy at Hounslow and Richmond Community Healthcare NHS Trust.


We are looking for talented and dynamic nurse for our new Hounslow Falls Prevention and Bone Health Service.

You will work as part of a multi-disciplinary team, including physiotherapists, occupational therapists, dietitian and therapy assistants. You will see patients contributing to a multi factorial falls risk assessment followed by a range of multi-disciplinary interventions. You will also work closely with colleagues in the acute Falls / Frailty clinic and community services. You will contribute comprehensive and specialist knowledge based on experience in general adults nursing in order to minimse the impact of frailty, promote wellbeing and prevent falls risks.

The service is delivered through multiple site clinics and home visits. The post holder will work flexibly and be mobile across the Hounslow Borough and in hospital settings.

Nursing assessment and interventions may include, but are not limited to:
• Medication reviews and referral for further medical assessment/ medication optimisation.
• Continence and its relationship with falls risk and prevention.
• Postural hypotension and systemic factors leading to falls risk.
• Frailty reviews in the context of Falls Prevention
• Provide comprehensive assessment and treatment or self-management plans, within scope of role, for clients who are at risk of, or worried about falling.
• Independently manage a caseload without direct supervision and to be professionally and legally accountable for all aspects of own work, including the management of clinical risk.
• Participate in pathway design and promotion of the service to encourage new sources and routes of referral to optimise the scope and impact of the service
• Assist the Team Leader in the provision of the service as necessary.
• Deliver intervention and education on an individual and group basis where required.
• Clinical assessment of factors affecting bone health and in the context of falls risk and frailty
