Summary

The Royal Free London NHS Foundation Trust & the North Middlesex University Hospital NHS Trust are to appoint a cross-site Consultant in HIV Medicine based at the Hampstead and North Middlesex site. This is a full-time (10 PA) substantive post. The successful applicant will be providing specialist outpatient HIV care in the Ian Charleson Day Centre (ICDC) and the Alexander Pringle Centre, and 12 weeks of in-patient HIV consultant cover (8 weeks based on Ward 11 South RFH and 4 at North Middlesex), including evening, overnight and weekend on-call (1:8). In addition, the post holder will enhance the service by taking a lead in developing research at the North Middlesex University Hospital HIV service and supporting the research portfolio at RFH. They will also be expected to help deliver the HIV TB service, and will be expected to contribute to service development generally. The post holder will perform clinics in HIV and also undertake in-patient duties and ward rounds.

The appointee will be responsible for inpatient HIV care, including ward rounds, outpatient clinics, and facilitating MDT meetings. They will oversee clinical research trials and supervise doctors in training. Teaching responsibilities include undergraduate and postgraduate education. Continuous Professional Development, annual appraisal, and revalidation requirements are expected. They will contribute to service development, national/local audits, and provide expert HIV outpatient care. On-call coverage for inpatient and ambulatory services is rotational. They'll provide clinical leadership, support the multi-disciplinary team, and develop care pathways for HIV patients. Efficient resource use and quality enhancement projects are priorities. They'll engage in clinical guidelines development, audit, and CPD activities, ensuring they meet revalidation expectations. Flexibility in clinical tasks within the allocated DCC is required.

The Royal Free HIV Service, part of the Royal Free London NHS Foundation Trust, is a globally recognised Centre of Excellence for HIV care. Catering to over 3000 HIV patients, it offers inpatient and outpatient services, including specialized clinics in various fields like viral hepatitis, TB, cardiology, and psychiatry. The department emphasizes network-level care and collaborates extensively within the North Central London Network. With a dedicated research team, it actively engages in clinical trials, notably for new antivirals and HIV infections. Additionally, the Royal Free has launched a vaccine trial research team, conducting large-scale prevention vaccine trials across various infections, building on its success with Covid vaccine trials.

In contrast, the North Middlesex University Hospital HIV service serves a diverse population, largely from Enfield and Haringey boroughs, with many patients from deprived backgrounds or born abroad. The Alexander Pringle Centre provides outpatient and inpatient HIV care, hosting specialist clinics like the Young Persons’ clinic and Antenatal clinic. Noteworthy achievements include leading the Opt-Out Blood Borne Viruses testing program and winning the Chief Executive Award at the 2023 North Middlesex Staff Awards.

A formal job plan will be agreed between the appointee and the clinical leads, on behalf of the Clinical Director, within one month after the commencement date of the appointee. If applicable, the job plan will be reviewed annually, following the appraisal meeting. The job plan will be a prospective agreement that sets out the appointee’s duties, responsibilities and objectives for the coming year. It should cover all aspects of an appointee’s professional practice including clinical work, teaching, research, education and managerial responsibilities. In addition, it should include personal objectives and details of the support required by the appointee to fulfil the job plan and objectives. The HIV Department conducts departmental job planning exercises as is common across the Trust.

The following provides a suggested schedule of the type of clinical and research activity and related activity components of the job plan. Based on 10 PA contract, there will be 7.5 Direct Clinical Care (DCC) sessions and 2.5 Supporting Professional Activity (SPA) sessions for this post.
