Summary


This is a senior role within the Integrated Discharge Team based primarily at Winchester but with the requirement to travel to Andover and Basingstoke in order to provide adequate nursing cover.

The team works with the multi-disciplinary teams within HHFT as well as with partners from the community to identify appropriate ongoing care for some of our most vulnerable adult patients and to manage the processes which expedite safe and efficient person centred discharge. The person will have knowledge and understanding of discharge pathways with a real interest and ability to facilitate conversations with system partners, wards, clinicians about appropriate onward care settings. They must be a skilled communicator with the ability to explain complex arrangements to patients and families as well as perform a teaching role on the wards.

The role includes liaison, teaching and negotiating with other professionals inside and outside the Trust, as well as with patients and their families. We need someone with a calm and confident approach, used to having challenging conversations introducing difficult concepts, and also with a good knowledge and ability with computer systems. The ideal candidate is usually someone who has tenacity and resilience and enjoys solving puzzles, someone who can work independently but also as an integral part of the team.
