Summary


Provide GP cover to a community hospital ward of 19 patients, predominantly admitted for rehabilitation and reablement following an acute hospital stay, or for end of life care. You will be the named responsible clinician for the ward and as such will review each patient at least weekly. On Willow Ward, you will form part of a multi-disciplinary team, working with the patient to enable discharge to their own place of residence as promptly as possible. You will be supported by a nurse practitioner and an experienced nursing team providing nurse-led care.

There are opportunities to be involved in service development of the hospital services, in line with system priorities. There is also opportunity to develop medical leadership through other parts of the Trust (eg learning from deaths, medical student training, quality improvement projects).

The role can be sessional/part time and hours can be flexible to suit the candidates work/life balance. Weekend working is possible.

In liaise with the senior nurses on the ward, provide senior medical decision-making to inpatients on community hospital ward.

Provide medical input into the MDT management of patients.

Contribute to advanced care planning (eg TEP) and end of life care.

Supervise and contribute to teaching and training of junior doctors (including GP trainees), medical students, ANPs and other AHPs

Liaise with patients, families and carers regarding their medical care

Liaise with other medical specialities eg Eldercare, Palliative care for specialist advice where needed.

Contribute to governance, incidents, learning, complaints and other process (eg legal) for the patients you have managed. Support the Clinical Director in the same.

Provide a holistic 'GP' view to patients' medical care while inpatients on the ward.

Review patients 'clerked' for admission by the ANP or junior doctors.

Undertake medication reviews to support safe prescribing in frailty and chronic disease.

Support the MDT to enable timely discharge of patients back to their own homes.

Contribute to a comprehensive geriatric assessment of each patient including treatment escalation plans and attempted resuscitation decisions.

Ensure that patients have had a medical review at least weekly and that patients approaching end of life have seen a doctor.

Supervise medical students, ANPs and other AHPs in their roles and studies (for example non-medical prescribing).

Attend medical emergencies in the hospital if required and requested (eg in MIU)

Support cross cover arrangements with the other ward in urgent circumstances.

Work with the medical team on the other ward, to support each other as a cohesive medical workforce across the inpatient wards.
