Summary

MAIN PURPOSE OF JOB As a member of the clinical team, to be responsible for providing high quality care and services, including minor illness, acute care, chronic disease management, and health promotion to the practice population within the PCN and to those that are housebound. POSITION IN ORGANISATION Accountable to PCN Clinical Director Reports to PCN Network Manager Liaises with GPs and Practice staff Liaises with Patients and Carers as necessary SCOPE OF JOB To review, assess, help manage and treat patients who are housebound, including those in care homes. This includes regular chronic disease reviews as well as acute problems. To assess, manage, treat, refer and/or signpost patients who attend the surgery and on home visits with undifferentiated or undiagnosed conditions relating to minor illness or minor injury. QUALIFICATIONS BSc in a training programme approved by the College of Paramedics Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11 Enhanced Clinical Assessment course Prescribing qualification desirable but not essential Health & Care Professions Council (HCPC) registration SKILLS AND EXPERIENCE Previous experience of autonomous practice in a community or practice setting Understanding of issues and developments in Primary Care Experience of management of chronic disease conditions in general practice, e.g. Diabetes, Asthma/COPD, Heart Disease, Dementia Excellent communication skills Able to build effective working relationships PART 2 JOB TITLE Paramedic DUTIES AND RESPONSIBILITIES assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN or practice team Perform specialist health checks and reviews Support the delivery of anticipatory care plans a process designed to support patients living with a chronic long-term condition to help plan for an expected change at some time in the future. As part of this to work alongside the current PCN proactive care team Perform and interpret ECGs Undertake acute and planned home visits on behalf of GPs, especially for local elderly or immobile populations maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice work with patients in order to support compliance with and adherence to prescribed treatments provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care attend practice multi-disciplinary meetings as appropriate TRAINING AND SUPERVISION Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Reflective practice and portfolio building. To impart knowledge and skills formally and informally to colleagues promoting peer review and best practice within the work environment. PROFESSIONAL Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations Maintain registration with Health & Care Professions Council (HCPC) GENERAL Attend and participate in practice and PCN meetings as required communicate at all levels across PCNs and other organizations, ensuring effective, patient-centred service communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care
