Job summary

Taunton Deane West PCN are looking for an experienced paramedic to join their forward thinking team.

To review, assess, help manage and treat patients who are housebound, including those in care homes. This includes regular chronic disease reviews as well as acute problems.

To assess, manage, treat, refer and/or signpost patients who attend the surgery and on home visitswith undifferentiated or undiagnosed conditions relating to minor illness or minor injury.

Main duties of the job

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN or practice team

Support the delivery of anticipatory care plans a process designed to support patients living with a chronic long-term condition to help plan for an expected change at some time in the future. As part of this to work alongside the current PCN proactive care team

Perform and interpret ECGs

Undertake acute and planned home visits on behalf of GPs, especially for local elderly or immobile populations

Prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

About us

Located in Somerset, Taunton Deane West Primary Care Network consists of Three GP practices, Luson Surgery, Wellington Medical Centre and Lister House. You will be working in a team consisting of Care Coordinators, Complex Care Nurses, First Contact Physiotherapists, Occupational Therapists, Pharmacists, CYP Mental Health Link Practitioners, Digital Data Coordinators, in addition to Practice staff.
