Summary


This post designed to support the administration function of the discharge hub and newly designed discharge process.
You will be based in the Bed Bureau and work with the Integrated Discharge Team to facilitate timely patient discharges by documenting accurate
patient information and agreed actions.

The main duty of this role is to support the discharge hub by:
• Updating and validating patient information.
• Ensuring documented actions are completed.
• Contacting patients discharged on pathway 0.
• Supporting the discharge team.
• Coordinating the discharge education program.
• Validating delayed transfer of care data.

• To provide a comprehensive and efficient clerical service within Patient Services as part of a multidisciplinary team. To ensure that all appropriate patient data is accurately recorded onto a specially developed database.
• Support the Emergency Admission of medical and surgical patients referred by General Practitioners and ED to the Trust.
• Ensure patients are admitted to the most appropriate site within the Trust.
• Liaise with Patient Service Coordinators, Trust nursing and medical staff to support the timely admission of urgent medical and surgical patients.
• Ensure correct details are given to ambulance control in order to ensure safe transport of patients to the appropriate destination.
• Support the ongoing development on new initiatives.
• Register new admissions to Freeman Hospital using the Patient Administration System efficiently.
• Provide technical support for Trust nursing staff using E record, the Trust’s bed management computer programme.
