Summary

We’ve got a brilliant opportunity available in the award-winning Live Well Wakefield team, as a Service Coordinator. If you’re passionate about helping local people to improve their health and wellbeing through personalised care initiatives, this is the role for you!

Live Well Wakefield is an established social prescribing and self-management service, offering person-centred advice to adults across the Wakefield District so they can access an extensive range of non-medical local options to improve their health, wellbeing, and independence.

What does a Service Coordinator do?

Coordinators work closely with colleagues across primary care, voluntary and community organisations and statutory services, to build effective working relationships in order to promote and develop Live Well Wakefield and local provision. So excellent communication and relationship building skills are essential! They are responsible for line management of a number of Social Prescribing Advisors and Link Workers, ensuring those colleagues are equipped with the skills, knowledge, and supervision necessary to allow them to succeed in, and enjoy their roles.

Coordinators also act as a specialist advisor to colleagues, offering social prescribing, and risk management advice to ensure our clients receive the best standard of care. They support the planning of team workloads, and ensure effective caseload management, whilst ensuring operational strategies are in place, and reviewed regularly to support service delivery and development. This is a great opportunity to exercise creativity in planning, implementing, monitoring, and evaluating new and existing workstreams.

Due to the key role of Live Well Wakefield in providing social prescribing and supported self-management services across the Wakefield District, and the continuing growth of the service, it is essential you are able to demonstrate ability to work under your own initiative and be able to adapt quickly to change.

Due to the nature of the position, we will require the successful candidate to complete an Enhanced DBS check. This post is fixed term until August 2025, to cover maternity leave.
