Summary
To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the ICC and primary care team. Assess, diagnose and treat specific conditions in accordance with agreed professional protocols and guidelines and manages programmes of care and treatment for patients with Long term Conditions including frailty. Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long-term condition. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols. Work with patients in order to support adherence to prescribing treatments. Actively engage patients and carers in management of their health, promoting health and wellbeing and self-care.
