Applications are invited for the post of GP with Special interest in Palliative Medicine.

This is an exciting opportunity to develop your clinical skills within a multidisciplinary setting that is supportive and covers all elements of palliative care.

The post holder will work at Meadow House hospice or in the community providing specialist palliative care services to the people of Ealing and Hounslow.

This is a part-time fixed term post for 6 months with an opportunity to extend. Hours of service are 9-5 Monday to Friday. The post currently has no on-call requirement. There is a plan to introduce a first on call service 9-5 Saturday and Sunday to which this post holder can contribute.

The number of sessions available can be discussed at interview.








Duties include:
• Working as part of a multidisciplinary specialist palliative care team that will contribute to the care of patients.
• Providing a holistic assessment of the needs of patients on admission or in the community
• Provide timely and evidence-based advice on management of symptoms and other concerns.
• Supporting other members of the specialist palliative care team in managing patients with complex needs and in handling difficult consultations.
• Take part in the appropriate referral of patients to other services, and in handover and sharing of appropriate information with other healthcare professionals.
• Attending weekly ward MDT, preparing all the relevant documentation beforehand and contributing actively to the presentation and discussion of individual cases.
• Ensuring that all documentation concerning ward patients is up to date and entered accurately onto the electronic patient record.
• Ensuring that all documentation for patients dying or being discharged from the ward is completed according to standard operating procedures and in a timely manner
