Summary
This is an exciting opportunity for the right candidate to join a newly expanding service. Joining the Learning Disability Intensive Support Service will allow you to truly make a difference in people's lives, using you skills to enhance life opportunities and ensure people live life well in the community. As a newly created post the successful applicant will have the opportunity to develop the role to fit the needs of the service.
This post covers the geographical area of Norfolk and Waveney.
The successful candidate will be part of an MDT, working together to ensure individuals in the community remain appropriately supported throughout a range of challenges. You will manage your own caseload, also communicating with the SLTs in the community LD teams to allow for seamless transitions between the teams.
The Intensive Support Service has received new funding to allow it to expand to better meet the needs of the LD population in Norfolk and Waveney. This exciting development will be further supported by regular supervision, a strong team work ethic and the opportunity to work with multiple teams and professionals across the county.

To be an active member of the integrated multi-disciplinary intensive support community team for people with learning disabilities.
To be responsible for a caseload of clients with complex needs, using evidence based/person centred principles to assess, plan, implement and evaluate interventions.
To provide clinical supervision and clinical leadership to other SLT staff as required.
To be part of the planning, development and evaluation of Specialist SLT services across the county to help ensure the consistency of high quality and efficient clinical services.
To deliver services in line with the appropriate clinical pathways/specification requirements.
To provide practice placements for SLT students, as required
