Summary


An exciting opportunity has arisen for 2 year fixed contract to work within renal to ensure a seamless service is delivered through screening patients and referrals. This will include patients who have chosen supportive care and those who are on dialysis and frail or considering withdrawal. Providing an assessment of patients’ care needs, and help develop, implement and evaluate care to provide clinical support to patients and their families/carers.

The post holder will develop a comprehensive patient-centred, nurse led, advanced care planning delivery pathway whilst working with MDT and supportive care within our expanding renal services. Providing specialist advice regarding assessment and support to facilitate high quality, individualised advanced care planning to patients and their families/carers attending the Trust. Deliver person centred care, frailty screening and comprehensive assessment for renal patients with or at risk of progressive and degenerative frailty. The role of the Specialist Frailty Practitioner will require someone who understands frailty and comprehensive assessments e.g. comprehensive geriatric assessment.

The post holder must possess fundamental leadership, advanced care planning and facilitation skills. The successful candidate will be required to demonstrate commitment to the strategic nursing agenda that delivers safe effective and compassionate care.

This post will require staff engagement and development, providing support, advice, and formal and informal teaching regarding frailty and supportive care.
