Key Responsibilities

• To oversee the management and prioritisation of a caseload. Conduct telephone inquiries, conduct in-person visits in individuals' residences or other suitable locations, and facilitate clinics.
• Determine eligibility for services as per the Care Act, considering daily living tasks and roles, as well as individuals' social, environmental, and psychological needs. Provide tailored advice and information, referring to other agencies as necessary.
• Conduct holistic assessments and take responsibility for meeting the needs of each assigned person, involving another worker only when necessary. Assess and provide assistive technology, equipment, and adaptations to meet needs and mitigate risks, ensuring safe and effective use.
• Arrange services to improve and maintain independence, particularly reablement services. Utilise clinical skills to assist individuals in identifying and managing their own risks. Conduct specialist functional assessments and provide specialised equipment and training when needed.
• Advise and facilitate minor and major property adaptations. Provide guidance on moving, handling, and hoist use. Assist individuals in accessing personal budgets when other support options have been explored.
• Clearly outline individuals' identified needs and outcomes in the support plan. Ensure the plan reflects the needs and outcomes of individuals and their carers. Identify cases requiring specialist assessments, such as mental capacity assessments, and conduct them according to professional qualifications and experience.
• Apply legal and professional knowledge to achieve positive outcomes while balancing protection, risk, and support. Serve as a lead professional in complex or disputed cases. Identify and take necessary intervention actions to protect at-risk individuals.
• Prepare court reports and mental health tribunal reports as required, and provide evidence during legal proceedings involving adults at risk. Deliver a customer-focused service adhering to "Customer Service Codes of Practice" and individual Service Level Agreements.

Educational achievements, Qualifications and Training

• Essential: Recognised Occupational Therapy Qualification and HCPC registration.
• Essential: Commitment to learning and development, with evidence of ongoing professional growth.
• Desirable: Best Interest Assessor or commitment to training.

Experience, Knowledge, Understanding and Skills

• Essential: Experience in needs assessments, safeguarding, and working with vulnerable populations.
• Essential: Strong interpersonal, communication, and IT skills.
• Desirable: Experience in adult safeguarding investigations.

Management of People and Personal Qualities

• Essential: Ability to adapt to change, handle pressure, and maintain a solution-oriented approach.
• Essential: Organised, receptive to feedback, and adept at prioritisation.
• Essential: Effective communicator, team player, and advocate for inclusive practices.

Strategic Awareness

• Essential: Understanding of organisational goals and ability to contribute to problem-solving.
• Desirable: Ability to identify systemic issues and propose solutions.
