Summary

The Discharge Planning (Co-ordinator) Support Worker will need competent I.T skills and the ability to use initiative to work to deadlines and prioritise workload and be comfortable with delivering teaching skills to patients, Relatives and

The successful candidate will be assessing and making appropriate and timely discharge plans for patients under the supervision of a registered nurse. Good communication skills, a pro-active approach towards discharge planning and the ability to work flexibly to meet the needs of the service is essential. They should be able to problem solve complex situations and recommend appropriate action.

There is a planned move for this service during 2025, when this role and base of work will relocate to Bournemouth site. Upon relocation of the role/service, excess mileage allowance / reimbursement will not apply, and any travel between the post holder’s home and work base will be classified as a commute. Any other changes unrelated to this, will be handled in line with Trust/National terms and conditions.
