Qualified social worker to complete review tasks predominantly within care home setting. Social worker to be part of an office based team in Sunderland.
Key Responsibilities


Provide person centred assessment of need at a level of complexity appropriate to the post, facilitating choice through the promotion of independence, as allocated by your line manager and where requested.
Work with the person or carer to develop an agreed care and support plan to achieve the identified outcomes to meet assessed eligibility need within resource allocation.
Refer, liaise, consult and work effectively with agencies and other professionals in order to meet identified needs.
Ensure care and support services are person centred, focused and promote the independence, overall safety and well-being of adults and where relevant children.
Promote and embed the effective delivery of strength based approaches to support planning, maximising on an individual’s strengths and community / family resources.
Provide professional support and information, guidance and advice on how their needs could be partly or wholly met by universal and other non-care services.
Proactively work towards embedding personalisation into practice including but not limited to individual budgets, direct payments, self-assessment and self-directed support. Practice and support planning will be focused on outcomes rather than service driven.
Undertake reviews of support packages as required ensuring the right level of support is delivered only for as long as is needed and at a reasonable price, advising the person and their carers’ of alternative services as appropriate.
Take professional responsibility for managing a case load, including adults with complex problems and whose circumstances may place their personal liberty or safety at risk.
Provide written reports and support plan to represent individual and carers needs and ensure you maintain a high level of data quality for all information recorded, ensuring that the information is timely, accurate and complete.

Qualifications:






A professional social work qualification such as Social Work Degree, MA in Social Work, Diploma in Social Work (CQSW, CSS) or equivalent
Current Registration with the HCPC
Full current driving licence or access to a means of mobility support

Knowledge:


Good working knowledge, understanding and application of the Care Act
Knowledge and understanding of other relevant legislation for adult social care, for example the Mental Capacity Act, Mental Health Act, Deprivation of Liberty Safeguards
Up-to-date knowledge and understanding of the Continuing Healthcare Framework and the ability to undertake joint health and social care assessments
Up-to-date knowledge and understanding of social care policy developments, best practice and current trends in health and social care

Experience:


Experience of providing a professional person centred social work service to people with care and support needs. This may include people with a learning disability, autism, older people and people with physical and mental disabilities
Experience of multi-disciplinary working
Demonstrable experience of undertaking complex assessments
Experience of personalisation and delivery though self-directed support
Experience of safeguarding frameworks, processes and procedures as well as managing complex cases
Experience of building relationships with community service providers and working in effective partnership to improve outcomes for individuals

Skills & Abilities:


Effective decision making skills which may involve considering risks
Effective time management skills and planning skills ensuring deadlines are met
Effective problem solving skills
Excellent communication skills with the ability to present complex information both verbally and in writing in a clear, concise manner to a range of audiences
Excellent case recording and report writing skills
Ability to undertake assessments, develop support plans and conduct reviews
Ability to contribute to the Councils aims and objectives within an overall framework that safeguards adults, promoting independence, health and wellbeing
Able to work outside of office hours as required as required by your contract of employment
