Salary: Band: 4, £23,152 p.a. plus a shift allowance (if applicable)

Hours: 37 hours, Full time although, part time would be considered, including early, late and weekend shifts

The Service

The Portsmouth Rehabilitation and Reablement Team is an integrated health and social care team, providing a short term Intermediate Care and Rehabilitation Service to vulnerable and disabled service users. The team consists of Rehabilitation and Reablement Assistants, Occupational Therapists, Physiotherapists, Nurses and Social Workers. The post holder will provide skilled support to maximise independence and to help the individual remain living in their own home for longer.

Your New Role

As an RRA you will assist clients in a range of daily living tasks, for example with transfers, mobility, washing and dressing, toileting, household tasks, meal preparation, social activity and access to the community. You will work to a rehabilitation philosophy empowering individuals to meet their personalised goals enabling them to be as independent as possible. With an understanding and implementation of safe moving and handling you will promote health and safety for service users working jointly and effectively with other professionals within the MDT including nurses, social workers, OT's etc. You will report promptly and discuss significant changes in service users to appropriate professionals. You will attend regular supervision sessions and participate in group supervision.

What you'll need to succeed

You will be patient and have an understanding of the needs of the client group, promoting and supporting individuals' rights and choices as well as being able to sensitively assist service users to maintain their independence. You will be need to accurately report and record information so you will need literacy and numeracy skills to enable you to do this. You will have well developed communication skills and the ability to converse at ease with customers and provide advice in accurate spoken English is essential for this post. Flexible and adaptable in order to meet the needs of the service you will be able to maintain confidentiality and be able to work as an effective team member. You must have a full UK driving licence and your own transport as your role will involve visiting service users across the city on a daily basis within a specific time span.
