More detail about the role The Community Therapy team work with people with a wide range of disabilities and acute or long term conditions striving to deliver excellent patient centred care. Our work is consistently varied and stimulating covering admission prevention and supporting discharge, and rehabilitation in a range of Community settings through medical, social and public health approaches. The Community Therapy Team works within the broader Health and Social Care team alongside colleagues from Community Nursing and Adult Social Care. We are a positive, progressive and proactive team and adapt to the changing needs of the patients we work with and the service demands. In support of Community based staff, our cluster has a fleet of shared pool cars that Clinicians are able to book to aid Clinical visits. Working Pattern: Up to 37.5 hours over 7 days.
