Summary


Exciting Opportunity in Children and Young People’s Mental Health

Join our new and growing team at LSCFT transforming children and young people's mental health. We are seeking 3 Pathway Coordinators to pioneer a role in our team.

This role is integral to our commitment to delivering high-quality, efficient, and effective care to our community. Data plays a crucial role in our operational and strategic decision-making processes, making its accuracy and reliability fundamental to our success. As a Pathway Coordinatoryou will be instrumental in maintaining these standards, directly influencing the quality of care provided and the future strategic direction of the organisation.

Your Role:
• Ensure data integrity within patient records, databases, and IT systems.
• Investigating and resolving complex data errors as a team
• Support in providing training to staff on data entry, best practices, and the utilisation of electronic health record systems.
• Oversee the monitoring of patient pathways from referral to discharge, identifying any potential delays.
• Work alongside our expert Pathway Officers, and Senior Operational and Clinical Leads.

Key Responsibilities:
1. Data Quality Assurance: Ensure data integrity within patient records and IT systems.
2. Data Verification and Validation: Analyse data for logical inconsistencies, errors, or missing information.
3. Error Identification and Correction: Detect and rectify data inaccuracies, collaborating with relevant staff to correct these.
4. Investigation of Data Errors: Provide specialised advice and support in resolving complex data quality issues.
5. Compliance and Reporting: Maintain data handling and reporting practices in alignment with legal, regulatory, and NHS standards, ensuring patient confidentiality and data protection.
6. Training and Support: Offer guidance and training to clinical and administrative staff on data entry standards, best practices, and the utilisation of IT systems.
7. Continuous Improvement: Participate in initiatives to enhancing data quality, contributing to the transition from reactive to proactive provision.
8. Monitoring of Patient Pathways: Monitor patient pathways from referral to discharge, identifying and reporting potential delays.
9. Presentation of Data: Report on data quality issues and patient pathways in weekly meetings, identifying challenges and proposing solutions.
10. Promotion of Data Quality Management: Elevate the importance of data quality management within the Trust, ensuring senior management is informed of issues and risks.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
