Summary

Job responsibilities MANAGEMENT OF PATIENT HEALTH AND ILLNESS To develop, implement and maintain practice based pharmacy services covering the core and additional areas as defined within the WYICB Practice Pharmacist service specification. To provide face to face and telephone reviews for patients within defined therapy areas and clinical competence as agreed with the GP partners. To provide prescribing support and advice to clinical staff within the surgery. To provide prescription support, training and advice to non-clinical staff within the surgery. To provide treatment advice and support to registered patients. To facilitate effective transfer of patient-care from external organisations to surgery through the appropriate processing of medication related communications eg. hospital discharge letters, consultant clinic letters. To undertake reviews of individual patients medication. To make recommendations for improving patient care and safety, to GPs and other healthcare professionals involved in the patients care. To utilise prescribing rights on an independent basis in the course of carrying out regular duties and responsibilities and within the pharmacists competence. To aid in the evaluation, development and implementation of guidelines protocols and formularies within the surgery. To contribute to the design and delivery of initiatives regarding the management of medicines within the practice. To be responsible for implementing these initiatives at practice level, monitoring changes made and ensuring that benefits are maintained in the future. To identify areas of clinical risk at practice level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of practice resources. To develop and maintain an effective network and working relationships with other members of the practice. To participate in multidisciplinary clinical meetings within the practice. To maintain own clinical and professional competence and be responsible for own Continuous Professional Development. To participate in clinical and medication audit in priority areas, such as QOF, NICE guidance, NSFs and other agreed target areas. To work in collaboration with individuals appointed in the ICB and other local practices on local priorities where this also will be of benefit to registered patients or has been agreed by the partners. To assist in the medication reviews and care plans of patients on the frailty register. To develop ideas which would benefit the practice in the longer term i.e. Research To support and ensure formulary adherence. To carry out monthly audits and feedback to partners at monthly meetings. To monitor and help control year to date growth of prescribing spend, to ensure this remains below the national average of 5%.
