Summary
The post holder is responsible for the delivery of practice nursing services and care to the practice population. Supported by GPs, Practice Manager and the Lead Practice Nurse, you will deliver care within your scope, focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities. You will work collaboratively with the whole team to meet the needs of patients and improve outcomes, supporting the delivery of policy and procedures and provide nurse leadership as required. The post holder will also strive to maximise the practices achievement of both Local and National Quality and Performance objectives.
