Summary

Peripatetic Ward Clerk
Band 2 -30 hours - Permanent
Monday to Friday, 8:00am-2:00pm & 1:00pm-7:00pm alternating weeks

We are looking for a highly motivated, enthusiastic and caring individual to join our Peripatetic Ward Clerk Team, who likes variety and is able to work flexibly, efficiently and independently to support a range of services.

The Peripatetic Ward Clerk role provides essential cover for the administrative roles that support our inpatient services here at Tameside Hospital; this will include; in patient Wards, Same Day Emergency Care & Early Pregnancy patient receptions, as well as general and specialist administration.

There are a variety of shift patterns between the hours of 7am and 9pm across a 7 day footprint and the successful candidate must be flexible to work across these hours/day including bank holidays etc.



Providing a comprehensive ward reception and administration service, the
post holder will, in collaboration with other ward staff, support patients,
relatives and other staff members as a first point of contact.

The clerk will update the Trust Electronic Patient Record (Lorenzo EPR) accurately and in real time whilst also ensuring patient case notes are available and appropriately maintained throughout each admission as well as performing other general office duties required by the service.

Confirm all recorded personal demographic details are correct and update for each patient admission/transfer/discharge

Ensure name bands are printed out for all patients

Provide verbal, written and electronic communications by receiving and passing on clear messages and seeking assistance where required.

Answering telephones and relay messages and enquires in a timely manner e.g. to nurse, doctors and patients.

Acts as first point of contact for the ward dealing appropriately with all communications and task allocated, including sensitive issues.

Liaises with patients, relatives and visitors in the department providing directions and information as required, obtaining the appropriate support when needed from the multi-disciplinary team.

Maintaining patient case notes throughout the admission ensuring efficient and accurate filing is complete on discharge or when patient is RIP.

Complete Discharge Tracker for each Discharge liaising with Clinical staff to ensure timely completion of Discharge Summaries.

Requesting and booking follow up appointments onto the clinical system. Communication and sending out appointment letters/discharge letters, completing electronic booking forms as required.

As directed by nursing staff, arrange for transport for Discharge/Transfer of patients.

Carrying out errands between departments

Maintaining stock and stationary levels including ordering, receiving and checking deliveries are accurate and reporting any discrepancies.

Establish and maintain effective filing systems

Assist with ward audits as required.

To participate in teaching, supervision and monitoring of new ward clerks as directed by Administration Manager.

Bed Bureau:

Ensure accurate and real time registration of each patient’s arrival, transfer, discharge or RIP recording within Lorenzo EPR.

Confirm all recorded personal demographic details are correct and update where needed.

Request and retrieve patient’s case notes upon admission.

Ensure efficient and accurate filing is complete on discharge where required.

Where patient is RIP during non-ward clerk hours bed bureau will attend the relevant ward and ensure all filing is completed before patient/case notes leave the ward.

Complete Discharge Tracker for each Discharge where ward clerk is not available, liaising with Clinical staff to ensure timely completion of discharge Summaries.

To participate in teaching, supervision and monitoring of new bed bureau clerks as directed by Administration Manager.

SECTION 2 - PLANNING & ORGANISATIONAL DUTIES

Plans and organises own day to day activities to achieve duties specified and to meet department requirements.

Provides cover for colleagues as directed by manager i.e. flexibility in working hours/days.

SECTION 3 – KNOWLEDGE AND SKILLS

The post holder will have computer skills.

The post holder will have good communication skills.

The post holder will have skills to communicate complex information to Nurse in Charge.

The post holder will contact patients, relatives, carers on the instruction of the Nurse in Charge if required to do so.
